Dear Participant,

This study is being conducted by Kathryn Curry an honours, undergraduate student under the supervision of Dr. Chris Shields, assistant professor in the School of Recreation Management and Kinesiology at Acadia University. The overall purpose of this research is to examine the exercise-related perceptions of Kinesiology students and compare these with those held by students enrolled in other degree programs.

Requirements of Participants

As a participant in this study, you will be asked to complete a series of brief questionnaires at two different time points – one today, and one a week from now. The first set of questionnaires will take approximately 20-30 minutes of your time, and the follow up questionnaire about 10 minutes to complete. The questionnaires will appear on screen after agreeing to the terms of this research. The information obtained from this study may add to the existing research on exercise-related perceptions. It may also serve to inform training in the field of Kinesiology.

Your privacy will be fully respected. The questionnaires you fill out will not be connected to you. Each questionnaire will have a personalized code (last letter of your first name, your day of birth, and the first letter of your last name); this is for the sole purpose of linking responses from questionnaire number 1 and 2. You will be asked for your e-mail address so that you may receive the second survey, however, your e-mail address will in no way be linked to your responses and will be deleted from the lab computer following the conclusion of the study. By providing your e-mail address you are not obligated to continue to participate in this study.  You may withdraw at any time.  Participation is completely voluntary.  Participants may withdraw their data up to the end of the data collection period by providing the researchers with their personalized code.  However, it will not be possible to withdraw results once the data has  been de-identified for analysis.

All information obtained in this study will be securely locked in the filling cabinet in the laboratory of the research supervisor, Dr. Chris Shields with all electronic data stored on a password-protected computer in the lab.  The data will be kept for a period of 7 years before being destroyed.

There are no known risks or side/effects associated with participation in this study. You will not directly benefit from participation in this research.  Confidentiality will be respected.  No information that discloses your identity will be released or published.  All information collected from participants in this study will be reported in group form in any report, publication or presentation resulting from this study.  The data will be securely stored on a password protected computer, of which only my supervisor and I have access. Please note that participants may choose to be entered into a draw to win one of five $20 gift certificates to a store of their choosing. Odds of winning this prize will be dependent on how many participants complete the study and choose this form of compensation. To be eligible to be entered into this prize draw you must complete the survey package at both time points of this study.  Your e-mail address is required in order to notify you if you are awarded the gift certificate or to receive your bonus point.  All e-mail addresses will be deleted following the completion of data collection

Participation in this study is completely voluntary and you may withdraw from the study at any time by choosing not to complete the questionnaire. In addition, you may leave any questions unanswered.  Participation (or not) will in no way impact your standing in your degree program.  If you choose not to participate you will no longer be eligible for entry into the prize draw.  If you have any questions, please contact me by e-mail at 098215c@acadiau.ca, or Dr. Chris Shields at (902) 585-1319 or by e-mail at chris.shields@acadiau.ca.
